If you have not seen Dr. Pleau previously or if this will be your first visit to this location, please schedule an Initial Visit. You will need to complete online intake and consent forms at least a day before your Initial Visit. Please review our Privacy Practices and read What to Expect before your appointment.
In order to use our online scheduler, you will need to provide a credit card number. By doing so, you authorize us to charge your card for services provided and for cancellation fees.
We require at least 24-hours notice for cancellations and rescheduling appointments. No shows and cancellations made with less than 24-hour notice will be subject to a one-time $35 cancellation fee. Late cancellation fees cannot be billed to insurance and are solely the responsibility of the patient or responsible party. Late cancellation fees must be paid prior to rescheduling your appointment.
